  • Medical Records Management:

  • Collect, organize, and maintain patient medical records and health information.

  • Ensure all records are accurate, complete, and up-to-date.
  • Retrieve records promptly for physicians, nurses, and authorized personnel.

  • Coding and Classification:

  • Assign medical codes (ICD-10, CPT) to diagnoses and procedures for billing and statistical purposes.

  • Work with billing and insurance departments for accurate claims processing.

  • Compliance and Confidentiality:

  • Ensure compliance with healthcare laws such as HIPAA (or local data protection laws).

  • Maintain patient confidentiality and secure handling of records.

  • Digital Recordkeeping (EMR/EHR):

  • Use electronic medical record systems (EMR/EHR) for data entry and retrieval.

  • Digitize paper records and manage document scanning.

  • Record Retrieval and Storage:

  • Organize the secure storage of physical and electronic records.

  • Implement record retention schedules and facilitate authorized disposal of outdated records.

  • Quality Control:

  • Regularly audit medical records for completeness and accuracy.

  • Coordinate with doctors and nurses to rectify any discrepancies.

  • Support Services:

  • Respond to requests for medical information from patients, legal entities, and external agencies, with proper authorization.

  • Assist in medico-legal case documentation and reporting.
