We are seeking compassionate and articulate professionals to join our International Voice Process Transitional Care Management (TCM) team.

The role involves making outbound calls to recently discharged patients to ensure a smooth transition from hospital to home, provide post-discharge guidance, and coordinate care. This is an opportunity to contribute to better patient outcomes and support continuity of care.

Key Responsibilities:

  • Make outbound calls to patients post-discharge to monitor recovery progress.
  • Educate and guide patients on medications, follow-ups, and health management.
  • Identify and escalate potential health concerns to clinical teams.
  • Maintain detailed and accurate patient records in CRM/EHR systems.
  • Adhere to HIPAA and healthcare data confidentiality standards.
  • Collaborate with healthcare professionals to ensure effective patient follow-up.

Requirements:

  • Education: Minimum High School Diploma / Graduate (Healthcare background preferred).
  • Experience: 1–2 years in International BPO/Healthcare/Voice Process (Freshers with strong communication skills can also apply).
  • Excellent English communication (neutral or US accent) and empathetic listening skills.
  • Comfortable working in night shifts (US shift).
  • Good computer and documentation skills.
  • Immediate joiners preferred.
