Job Title : Clinical Lead ()

Location : Mumbai

JOB PURPOSE

This role is responsible for overseeing and ensuring proper patient monitoring, particularly in high-need areas, to ensure high-quality care is consistently provided. This role involves the coordination of patient assessments, family counseling, incident reporting, quality assurance, team management, and continuous staff development. The Patient Care Coordinator plays a key role in ensuring optimal patient care outcomes through collaborative efforts with internal and external healthcare professionals and effective communication with family members.

RESPONSIBILITIES

1.
Patient Monitoring & Care:

  • Oversee patient care, especially in high-need areas, ensuring proper patient monitoring.
  • Coordinate patient assessments by Advanced Team Leads (ATL) and In-House (INH) teams, ensuring quality care is delivered.
  • Facilitate stakeholder meetings and build strong relationships with internal and external doctors (within and outside the hospital).
  • Ensure readiness of new products from a clinical perspective (e.g., Dialysis at Home, Mother & Child services).
  • Conduct daily clinical team meetings, discussing goals of care with primary and treating doctors.
  • Perform home visits and out-of-hours (OOH) patient assessments, reviewing patient progress regularly.
  • Identify red flags and take immediate action to address safety concerns.

Family Meetings & Counseling:

  • Organize and conduct family meetings to update family members on patient progress and care plans.
  • Provide counseling and guidance to family members regarding patient care and expected outcomes.

3.
Quality Assurance & Incident Reporting:

  • Maintain high standards of patient care and identify opportunities for quality improvement.
  • Document incidents, prepare detailed reports, and facilitate investigations for timely resolution.
  • Monitor patient care trends and report findings through data analysis and dashboards.

4. Team Coordination & Meetings:

  • Lead and actively participate in daily team meetings, ensuring continuous communication on patient progress and care issues.
  • Engage in case reviews, implementing corrective measures to improve patient outcomes.
  • Facilitate regular coordination with doctors and care teams to align treatment plans and address concerns.

5. Training & Development:

  • Organize and facilitate training sessions for nursing staff, including refresher courses and the New Hire Training Program (NITP).
  • Coordinate On-the-Job Training (OJT) for team leads (TL), assistant team leads (ATL), nursing supervisors, and nurse educators.
  • Ensure continuous upskilling for nursing staff and leadership with relevant training modules.

6. Recruitment & Interviews:

  • Conduct interviews for nursing roles, including ATL, nurse educator, nursing supervisor, and related positions.
  • Assist in the recruitment process to ensure that staffing requirements are met.

7. Palliative Care:

  • Develop, manage, and oversee palliative care projects, processes, and related training modules.
  • Coordinate dashboards to track the progress of palliative care patients.

8. Root Cause Analysis (RCA) & Corrective Action Plans (CAPA):

  • Participate in Root Cause Analysis (RCA) for incidents affecting patient care and safety.
  • Support the development and implementation of Corrective Action Plans (CAPA) to address identified issues and improve care delivery.

9.Data Management & Dashboards:

  • Collect, analyze, and present data on patient care quality, incident trends, and overall performance.
  • Develop and manage dashboards displaying key performance indicators related to patient care, training outcomes, and incident management.

Qualifications : MBBS

Experience :
5+ years of experience
