Role Overview:
The Care Manager is responsible for overseeing the coordination of care and services provided to patients or clients, ensuring they receive high-quality, patient-centered care. This role involves working closely with patients, their families, and interdisciplinary teams to assess, plan, implement, and evaluate care plans. The Care Manager will ensure that all services are delivered effectively, efficiently, and in compliance with legal, regulatory, and organizational standards.
Key Responsibilities:
1. Care Coordination and Management:
- Develop individualized care plans based on patient assessments, goals, and preferences.
- Coordinate with healthcare providers, families, and community resources to ensure the delivery of appropriate services.
- Monitor patient progress and adjust care plans as necessary.
- Oversee patient admissions, transfers, and discharges, ensuring a smooth transition in care.
2. Patient Advocacy and Support:
- Act as a liaison between patients, families, and healthcare professionals to ensure clear communication and understanding of the care plan.
- Provide guidance and support to patients and their families regarding treatment options, care procedures, and available resources.
- Advocate for the patient's needs and ensure their rights and dignity are maintained.
3. Quality Assurance and Compliance:
- Monitor and evaluate care outcomes, ensuring continuous improvement and patient safety.
- Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with HIPAA or relevant privacy laws.
4. Reporting and Documentation:
- Document all care-related interactions and decisions in the patient's record, including any changes to care plans, patient conditions, and care team communications.
Required Skills and Qualifications:
- 2-4 years of experience in clinical role (preferred)
- Strong interpersonal, communication, and problem-solving skills.
- Excellent coordination and organizational skills to manage multiple stakeholders.
- Ability to manage multiple tasks, prioritize, and work under pressure.
- Compassionate, empathetic, and patient-centered approach to care.
