Track and locate STEMI referral forms for all **Reported STEMIs** logged in Cardionet app & STEMI registry - Tag and categorise all Reported STEMI cases (with missing referral forms) under the below category - Verify the STEMI referral form received from spokes for the Reported STEMIs - Coordinate with Spoke (Through FSE or Directly) for missing referral form - follow up call should be made as quickly as possible after STEMI alarm with support from spoke coordinator (Can be automated) - Create daily lost to follow up report (due to No Data or incomplete data shared from spoke) - Reported STEMIs with - "STEMI" tag / Incomplete referral form - Coordinate with Hub (Through FSE or Directly) for all treatment forms for patients treated under MJPJAY - Receive it through Cardionet/ WhatsApp - Coordinate with Patients (over call) for all treatment received outside the STEMI Hub & Spoke network - Perform data entry for all STEMI referral and treatment into the STEMI Registry. - Collect and upload patient discharged summary of all patients received with treatment - Call and retrieve any missing information from the patient or the patient's family members. - Prepare and submit weekly/ monthly report of patient outcome for the assigned cluster **Job Types**: Full-time, Permanent Pay: ₹20,000.00 - ₹22,000.00 per month **Benefits**: - Health insurance Schedule: - Day shift **Experience**: - total work: 1 year (required) Work Location: In person Expected Start Date: 01/02/2025
