**Job Summary**:
**Key Responsibilities**:

- Coordinate and schedule physiotherapy and home care services for patients based on clinical priorities and availability.
- Act as a central point of contact between patients, families, physiotherapists, home care nurses, and healthcare providers.
- Monitor patient care plans to ensure services are delivered according to medical advice and regulatory standards.
- Maintain accurate and timely documentation of patient care, therapist schedules, service delivery, and case progress.
- Ensure compliance with all healthcare regulations, privacy standards, and organizational protocols.
- Assist with staffing coordination and coverage planning for physiotherapists and home care providers.
- Evaluate service quality and patient satisfaction; address concerns and implement improvements as needed.
- Support discharge planning by coordinating continued home care needs post-hospital or clinic discharge.
- Participate in multidisciplinary meetings and contribute to care planning and quality assurance initiatives.
- Maintain and update service delivery logs, billing records, and referral tracking systems.

**Qualifications**:

- Bachelor's degree in Physiotherapy, Nursing, Healthcare Administration, or a related field (Master’s preferred).
- Registered/licensed in relevant clinical field (if applicable).
- 3+ years of experience in physiotherapy coordination, case management, or home health services.
- Strong knowledge of rehabilitation therapy and home care service delivery.
- Familiarity with electronic health records (EHR) and healthcare scheduling software.

Pay: ₹16,399.15 - ₹25,616.71 per month
