To collect basic identification data from the patient and record it on the patient medical file.
- To register new and old (follow-up) patients and arrange appointments.
- To help and direct patients in the outpatient clinic.
- To collect investigation reports (laboratory, x-ray and other investigation) and to arrange and mount them in the appropriate records.
- To receive patient records outpatient, inpatient and emergency patient, and to arrange and file them in the appropriate location.
- To retrieve records and send them to the required department.
- To be capable to work in all units of the medical record department as well as new registration, follow-up registration, admission office, and to carry out the work of that units as per recommended procedures.
- To strictly observe instructions given in the procedure manual to carry out assigned duties and responsibilities.
- To maintain confidentiality of patient information and prevent disclosure to unauthorized persons.
- To provide medical record services including reception, registration, appointments and inpatient, outpatient and emergency department filing and retrieving systems.
- To evaluate documentation for deficiencies in the outpatient and inpatient medical files and to arranging for completion of records with the cooperation of medical and nursing staff.
- To feed patient care information into the computer for processing, storage, and retrieval when required.
- To assist medical education and medical research programs by providing required information.
- To protect medical records especially medico legal cases from unauthorized disclosure so as to maintain confidentiality.
- To perform any other work related to medical records as instructed by management.
- To assemble and process records as per the established procedures.

**Salary**: ₹15,000.00 - ₹20,000.00 per month
