**Key Responsibilities**:

- **Patient Support and Advocacy**:

- Provide empathetic, non-judgmental support to patients and their families.
- Serve as a patient advocate to address concerns, preferences, and needs within the healthcare system.
- **Information and Education**:

- Explain medical diagnoses, treatments, and procedures in an understandable manner.
- Ensure patients fully understand their care plans and medication instructions.
- Educate patients on lifestyle changes, preventive care, and self-management techniques.
- **Emotional and Psychological Support**:

- Offer counseling to patients dealing with chronic illnesses, trauma, or stress related to their health conditions.
- Provide grief and loss support to patients and families when necessary.
- **Coordination and Resource Navigation**:

- Assist patients in scheduling appointments, obtaining referrals, and accessing healthcare services.
- Connect patients with community resources, financial aid programs, or support groups.
- **Compliance and Follow-Up**:

- Monitor patient adherence to treatment plans and encourage active participation in their healthcare.
- Conduct follow-ups to ensure continuity of care and address any emerging concerns.

**Qualifications**:

- Bachelor’s degree in counseling, psychology, social work, or a related field (Master’s degree preferred).
- Certification in counseling or patient advocacy is a plus.
- Strong knowledge of healthcare systems and medical terminology.

**Skills and Competencies**:

- Excellent communication and interpersonal skills.
- Empathy, patience, and the ability to build trust.
- Problem-solving skills and the ability to work under pressure.
- Organizational skills to manage multiple patients and priorities.

**Education**:

- Bachelor's (preferred)

**Experience**:

- Counselling: 1 year (preferred)
