**Job Summary**:
Care Bridge Home is seeking a dedicated and experienced **Home Care Coordinator (BSc Nurse)** to oversee and manage home care services in Thiruvalla. This hospital and community-based role requires clinical skills along with strong **leadership, management, and coordination skills** to ensure high-quality patient care and effective service delivery.

**Key Responsibilities**:

- **Operational Management**: Oversee the daily operations of home care services, ensuring compliance with healthcare regulations and organizational policies.
- **Care Planning & Assessment**: Develop, implement, and evaluate individualized care plans based on clients’ specific health needs.
- **Team Supervision & Training**: Recruit, train, and manage caregivers, ensuring they deliver high-quality care. Conduct regular performance evaluations and training programs.
- **Coordination & Communication**: Work closely with healthcare professionals, families, and clients to provide holistic and effective care.
- **Client & Family Support**: Address concerns, manage complaints, and ensure client satisfaction with professional and timely resolutions.
- **Record Keeping & Compliance**: Maintain accurate records of client care, staffing, finances, and operational activities while ensuring compliance with regulatory requirements.
- **Community Engagement**: Build and maintain relationships with hospitals, doctors, and other healthcare providers to generate referrals and expand services.
- **Policy & Compliance Management**: Ensure adherence to organizational policies, ethical standards, and industry regulations.

**Requirements**:

- **BSc Nursing** with a minimimum of 1 year experience
- Strong leadership, management, and problem-solving skills.
- Excellent **communication skills** in **Malayalam and English**.
- Willingness to **travel** and manage both hospital-based and community-based responsibilities.
- Must be **locally available** in Thiruvalla.

Pay: ₹22,000.00 - ₹335,000.00 per month

Schedule:

- Day shift
- Monday to Friday
- Weekend availability
