**Job Summary**:

- We are hiring a HCC Medical Coder to join our team in Koramangala, Bangalore.
- You will ensure coding compliance and contribute to the accuracy of risk adjustment data.

**Key Responsibilities**:

- Review patient medical records and assign appropriate HCC diagnosis codes.
- Ensure the accuracy of ICD-10 coding for chronic conditions as per HCC models.
- Perform coding audits and quality checks to maintain coding accuracy.
- Collaborate with healthcare providers to clarify documentation and resolve coding issues.
- Stay updated with coding guidelines, HCC risk adjustment models, and CMS regulations.
- Participate in ongoing training to enhance coding skills and maintain certification standards.
- Strong knowledge of ICD-10 coding and ability to accurately code diagnoses based on medical records.
- Key Skills: In-depth understanding of medical coding systems (ICD-10, HCC).
- High attention to detail and analytical skills in reviewing and interpreting medical records.
- Familiarity with compliance guidelines and coding standards (CMS, HCC models).
