**Job Description: Staff Nurse - Home health Care**

**Position Overview**: We are seeking a compassionate and dedicated Staff Nurse to join our home care team. As a Staff Nurse in home care, you will play a crucial role in providing high-quality healthcare services to patients within the comfort of their homes. You will be responsible for delivering personalized nursing care, monitoring patient progress, and ensuring their overall well-being.

**Key Responsibilities**:

- Provide skilled nursing care to patients in their homes based on individualized care plans.
- Conduct comprehensive assessments of patients' health status and collaborate with healthcare professionals to develop effective care plans.
- Administer medications and treatments as prescribed by physicians and monitor their effects.
- Educate patients and their families on disease management, treatment options, and lifestyle modifications.
- Perform wound care, IV therapy, catheter care, and other specialized nursing procedures as required.
- Maintain accurate and timely documentation of patient care activities and progress notes.
- Monitor and report changes in patients' conditions to the clinical team and primary healthcare providers.
- Ensure compliance with all regulatory requirements, standards of care, and company policies.

**Qualifications and Skills**:

- Valid nursing license in the state.
- Proven experience as a nurse in a clinical or home care setting is preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate attitude and empathy towards patients and their families.
- Reliable transportation and willingness to travel to patients' homes within designated service area.
