Position: Medical Social Worker

Qualification: MSW.

Should be fluent in English, Marathi and Hindi. Posses excellent communication and co-ordination skills. Should posses good maturity to deal with patients & relatives

Experience: Experience 1 to 3 years from hospital industry.

Shifts : General

Key Responsibilities:
Reviewing new admissions, assessing patients' physical and psychological needs, and conducting screenings, Creating personalized plans, including treatment and discharge plans. Offering counseling, psychotherapy, and grief counseling, and providing emotional support to families. Helping patients understand their financial and emotional resources and educating the community about health care programs. Assisting with the discharge process, and helping patients transition from the hospital to the community. Coordinating the care of patients with complex needs Co-ordinating with patients, consultants, various departments right from admission to discharge/ post discharge related to patients, Working towards enhancing patient satisfaction and followup with patients.

Pay: ₹20,000.00 - ₹25,000.00 per month
