The Primary day to day responsibilities of a CLINICAL PHARMACIST would be as follows:

- To ensure compliance of MOM (Management of Medication) as per NABH protocols

Daily Ward rounds to ensure safe medication practice and use on all patients - Medication chart auditing. “Right drug for the right patient at right dose, route, frequency and duration.

Past medication history interview and action to prevent therapeutic duplication with the existing plan, treatment and discharge.

Preventing under-treatment for certain conditions left unseen.

To act as a first port of call for all medicines related issues.

Suggest alternative medicine in case of which has therapeutic benefits over existing one.

Highlight any serious drug interactions in case sheet and inform the Consultant / DMO with standard proof and justification and suggest a method/alternative

Monitoring rational use of antibiotics, dose calculation according to patient condition and ensuring right drug according to sensitivity report.

Dosage adjustment in certain conditions as per Serum Creatinine level, liver dysfunction and according to INR.

Educate patients and caregivers on proper medication use, especially for complex cardiac regimens.

Provide training for nurses and physicians on new drugs, guidelines, and safety protocols

Disseminate Drug information to Doctors, Staff Nurses and Healthcare professionals as and when required.

Medication error Daily error report submission to Quality department and also to Senior Consultants.
