1. Coordinate Monthly Doctor Consultation Schedule:
- Work with the Program Coordinator to develop a monthly calendar for street
consultations
- Assign consultation slots to targeted zones based on chronic case load and referrals
- Confirm availability (internal/external) 7 days in advance
- Ensure transport/logistics are arranged for street visits
- Communicate consultation schedule to field teams and update shared calendars
2. Provide Medical Consultations and Review Treatment Plans
- Conduct field consultations weekly or as per schedule
- Review vital signs, symptoms, history, and available diagnostics
- Document diagnosis, treatment, and follow-up plan clearly in the field record
- Provide prescriptions in local language with pictorial guidance if needed
- Review and approve treatment plans suggested by CHNs, ensuring alignment with
standard protocols
3. Conduct Follow-up Visits for Chronic Cases and Provide Emergency Support
- Prioritize follow-up for high-risk chronic cases (e.g., diabetes, hypertension,
epilepsy)
- Conduct in-person follow-up in the community for complex, unstable, or relapsing
clients
- Offer phone/video call support for CHNs managing stable cases
4. Train and Guide CHNs and Nalam Workers on Medical Aspects
- Conduct monthly training sessions on common clinical issues (wound care, chronic
disease signs, medication counseling)
- Review challenging field cases in team meetings for shared learning
- Review and validate the documentations done by them in physical records as well as
MIS
- Provide practical demonstrations during joint field visits
- Maintain a field training log and track individual competency growth
- Offer corrective feedback in a supportive manner
5. Ensure Rational Treatment and Follow-Up
- Follow WHO and national guidelines for prescriptions and clinical care
- Avoid polypharmacy and unnecessary antibiotic use
- Ensure lab referrals are justified and tracked to completion
- Review and refine follow-up protocols for chronic clients (e.g., BP/Glucose
monitoring frequency)
- Conduct quarterly audits of prescriptions and follow-up logs
- Ensure medical logs are maintained properly and periodical stock check
