**Job description**

BDS, BAMS, MBBS. Medical officer for Gynaec and IVF hospital.

Initiates the medical record by creating and processing the patient care record folder. Patient counseling/ doubt clearing/ coordinating and Gathering patient demographic and personal information.

Maintain quality and accurate records by following hospital procedures. Ensures medical records are assembled in standard order and are accurate and complete.

Ensure patient charts, paperwork and reports are completed in an accurate and timely manner. Communicate with Medical Staff, health care team members and patient departments. When required ward manage/support.

Keeps health care providers informed by communicating availability or unavailability of the record. Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.

Maintains patient confidence by keeping patient records information confidential. Enhances medical records and hospital reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
