Patient Care Co-ordinator
Patient care coordinators are liaisons between patients and the healthcare system. As a patient care coordinator, you would ensure that patients receive the care they need and that they understand their medical condition, medications, and other instructions.
**Responsibilities and Duties**
**Responsibilities**:
- Perform regular follow up activities with patients and help the patient to understand the treatment.
- Provides coordinated care to patients, pre-surgery and post-surgery instructions.
- Establishing rapport with patients and coordination between doctor and patients.
- Evaluating the outcome of the care provided.
- Maintain patient database and managing incoming phone calls of patients, solving their queries.
- Follow all policies and procedures and other duties assigned.
**Required Experience, Skills and Qualifications**
**Desired skills and qualification**:
- Knowledge of healthcare sector and delivery system.
- Experience of at least 2-5 years in health care sector.
- Effective communication skills in Hindi and English.
- Ability to work independently as well as in a team environment.
- Ability to learn and adapt new system.
- Good in MS word & Excels
