**Physician / Diabetologist**

**Practo - Chronic Care : Diabetes Remission Program
A great opportunity has arisen for an enthusiastic physician with a passion for diabetes care to work in a full time role with the Diabetes Remission Program of the Practo - Chronic Care Platform.

The physician will join an expanding team of HCP working on a Diabetes remission program to begin with and expand their role to the management of other chronic health care conditions, in the wider Chronic Care Platform.

**Qualifications**

**Essential**:

- MBBS
- Minimum 5 years of experience in general medicine/diabetology or medical subspecialties either in a hospital or OP setting
- Competent in chronic disease management
- Competent in diabetes management
- Fully registered to practice: KMC / IMC registration

**Desirable**:

- MD or DNB in general medicine/ endocrinology
- Proficient with one or two regional languages (Kannada/ Telugu/ Tamil/ Hindi)
- Experience with diabetes health-care technology i.e. CGM / sensors
- Able to demonstrate dedication to diabetes as a subspecialty in training, education.

**Job responsibilities:
The Physician will be expected to work **independently **in the following roles.

Periodic supervision and training will be provided by the medical lead on a weekly basis
- Assess patient eligibility in program along with screening team
- Assess comprehensive medical history and identify underlying other medical complications of diabetes and other chronic disease that may impact patient journey in program
- Work jointly with the multidisciplinary team (MDT) to deliver a customized health care plan to the patient and review these plans regularly throughout the program.
- Review, interpret and intervene on investigations for chronic disease - for eg comprehensive blood profile including lipid profile, hba1c, ECG, BP etc.
- Medication management: Periodically review and titrate medications for diabetes including insulin
- Provide education to patients to recognize and prevent hypoglycemia
- Ability to identify red flags, high risk patients and escalate concerns.
- Undergo training in CGM: To be able to recognize patterns, averages, red flags, and customize targets for individual patients.
- Educate patients in interpreting CGM and help translate this education into self management.
- Review CGM output of individual patients on a bi-weekly basis and tailor medication withdrawal appropriately.
- Be available to the dietician/mentor on a day to day basis for trouble-shooting issues
- Recognizing diabetes remission and enabling tapering of program jointly with MDT
