Nutritionist for Patient Counselling

**Key areas of responsibility**:

- Provide counselling related to Cidmus, its usage, side effects and treatment adherence.
- Provide counselling and psycho-social support to heart failure patients and their families.
- Develop individualized care plans and schedules to help manage their condition.
- Assist patients and their families with understanding their condition and how Cidmus can help them Educate patients and families on lifestyle and medical management of heart failure.
- Coordinate with all stakeholders to ensure all aspects of patient care are addressed.
- Track patient progress, document patient care, and maintain patient records.
- Provide emotional support to family members and other caregivers
- Work with healthcare systems to ensure continuity of care for patients

**Qualification**:

- Bsc Food & Nutrition / Msc Food & Nutrition
- Language -English(mandatory)+ Any Regional Language

**Salary**: ₹20,000.00 - ₹27,000.00 per month

- Srinagar, Jammu and Kashmir: Reliably commute or planning to relocate before starting work (required)

- Bachelor's (required)
