**Responsibilities**:

- Provide comprehensive medical care to geriatric patients in outpatient and/or inpatient settings.
- Conduct thorough assessments and develop individualized care plans tailored to patients' unique needs and preferences.
- Manage chronic medical conditions, acute illnesses, and age-related health issues in collaboration with other healthcare team members.
- Coordinate care transitions, including hospital admissions, discharges, and post-acute care arrangements.
- Collaborate with specialists, therapists, and other healthcare professionals to optimize patient outcomes and quality of life.
- Educate patients, families, and caregivers about geriatric health issues, preventive measures, and treatment options.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.

**Qualifications**:

- Medical degree from an accredited medical school.
- Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Valid medical license in the state of practice.
- Proven experience in providing primary care to geriatric patients.
- Excellent clinical skills, including proficiency in managing complex medical conditions and geriatric syndromes.
- Strong communication, interpersonal, and teamwork skills.
- Commitment to delivering patient-centered care with compassion and empathy.
- Ability to adapt to changing healthcare environments and work collaboratively in a multidisciplinary team.
