Business/ Financial
- Responsible for safe storage of patient personal files containing all documents for a period of years as per NABH standard.
- Maintains master patient index by completing assigned portion of daily audit trail.

Operations
- Maintains medical records operations by following policies and procedures; reporting needed changes.
- Keeps health care providers informed by communicating availability or unavailability of the record.
- Maintains patient confidence by keeping patient records information confidential.
- Maintains quality results by following hospital standards
- Completes medical record by reviewing information
- Resolves medical record discrepancies by collecting and analyzing information.
- Monitoring EMRD system on monthly basis
- ICD coding of medical diagnosis of discharged patients

Customer
- Notifying PCS for record deficiencies, tracking outstanding records, notifying medical director of physicians in jeopardy of losing admitting privileges
- Coordinate with the local register at municipality office for birth certificate generation within time frame.
- Gathers patient information by collecting demographic information from them and other variety of sources available. (information is collected from the patient only)

People
- Coordinates with other team members for receiving/ sharing accurate information

« Any other portfolio that may be entrusted by the management from time to time.

**Job Types**: Full-time, Regular / Permanent

**Salary**: ₹9,954.03 - ₹27,854.80 per month
