Job Description: We are looking for a qualified Physician Assistant or a Care Coordinator to join our healthcare team and play a key role in monitoring patients through the KeeboHealth platform. The ideal candidate will use their clinical know-how and expertise to monitor patient vitals, and provide Operational support. Additionally, the candidate will be responsible for qualifying potential candidates by establishing and understanding their needs. This role requires excellent communication skills and the ability to work collaboratively within a digital healthcare setting.

Key Responsibilities:

  • Regularly monitor patient data and vitals using the KeeboHealth platform, ensuring timely interventions and follow-ups.
  • Coordinate patient care plans and ensure adherence to prescribed treatment protocols.
  • Assist in clinical operations, including patient onboarding, documentation, patient follow-ups (for key areas like Vitals, appointments, medication etc.)report generation, and patient close-out.
  • Serve as the point of contact for patients, addressing any non-clinical questions and concerns related to the products.
  • Work closely with doctors to manage day-to-day patient care activities.
  • Coordinate regular virtual calls between the medical lead (Tricog Clinical Team) and patients.
  • Generate patient feedback and work with the Product Manager to translate this into actionable insights and product features
  • Work with the BD team to talk about the KeeboHealth benefits to generate the interest of patients/caregivers and onboard them on the platform
  • Coordinate with the hospital cardiologist and ensure that the patient is in the loop for all follow-up visits and in case of ad-hoc schedule of consultations due to off-track vitals

Requirements:

  • Bachelor's degree in Biology or allied field.
  • GNM / Nursing Experience preferred but not mandatory.
  • Experience monitoring cardiac patients is highly preferred as a Care Coordinator but is not mandatory.
  • Must have strong communication and interpersonal skills, with the ability to provide virtual care.
  • Comfortable with using digital health platforms and technology for patient management.
  • Ability to work independently and as part of a remote healthcare team.
  • Ability to work in a start-up environment with a clinical setting and managing KOLs and other key stakeholders
  • Speaks at least fluent English, Hindi and Kannada
  • Flexible to travel across Tricog offices and customer/patient locations, as needed
