We are seeking a highly organized and passionate individual to join our team as a Healthcare Program Coordinator. This role will support patient engagement, access to care, and coordination with treatment centers across multiple healthcare initiatives.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare providers, and community partners. A strong understanding of healthcare systems and policies is also essential for success in this role.
Responsibilities include:
- Supporting TB and Rheumatology patients with treatment adherence, follow-up, and emotional counseling.
- Assisting patients in applying for government and non-government aid programs at both state and central levels.
- Helping patients and caregivers with documentation, form submissions, and navigating official processes for availing treatment support.
- Creating and maintaining a repository of state and national-level patient assistance schemes and health entitlements.
- Tracking and following up with patients regularly to ensure continuity of care and linkage to appropriate support programs.
- Coordinating with hospital social work departments, government agencies, and NGOs to ensure patient needs are addressed.
- Identifying and building a local donor network, including individuals and community-based groups, willing to support patients financially or in-kind.
- Maintaining accurate patient records, gathering field-level insights, and contributing to regular reporting and documentation.
- Assisting in field logistics, coordination with healthcare staff, and support day-to-day project operations.
Requirements:
- Master's degree in Social Work, Public Health, or a related field.
- Graduates from institutions like TISS, PHFI, or similar are strongly encouraged to apply.
- 2-4 years of relevant experience in healthcare-related social work or public health programs.
