Job Title: Medical Document Specialist
This role is responsible for utilizing advanced technologies to support healthcare providers in optimizing their time and resources.
The ideal candidate has experience with electronic health record (EHR) systems, specifically in scribing visitation notes into patient charts while maintaining a deep understanding of multiple provider workflows.
Key Responsibilities:
- Review physician dictations and transcribe clinical notes into EMR/Billing systems.
- Prepare and assemble medical record documentation/charts for physicians.
- Ensure medical record compliance through self-documentation attestation.
- Update patient history, physical exam, and other pertinent health information in the patient's record.
- Prepare and send all documentation to physicians for review and approval via authentication of detailed data entry and facility-specific procedures.
- Maintain accuracy of lab results and screening procedures.
- Comply with hospital and medical facility policies, including those related to HIPAA and Joint Commission.
- Perform additional clerical duties and tasks as assigned to improve provider productivity and clinic workflow.
- Assign codes to diagnoses and procedures using ICD (International Classification of Diseases) and CPT (Current Procedural Terminology) codes.
- Verify code accuracy and sequencing according to government and insurance regulations.
- Communicate with other clinical staff regarding documentation and provide clarity on any unclear or insufficient documentation.
- Search for information in cases where coding is complex or unusual.
- Review patient charts and documents for accuracy and completeness.
- Evaluate and code previous day's batch of patient notes.
- Ensure that all codes are current and active.
Required Skills and Qualifications:
- 3-5 years of experience as a live medical scribe.
- Advanced administrative writing skills.
- Reporting and organizational skills.
- Record-keeping and Microsoft Office proficiency.
- Professionalism, confidentiality, and strong communication skills.
- Typing speed and accuracy.
- Bachelor's degree with pre-health career track preferred.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Familiarity with ICD-X codes and CPT procedures.
- Skillful operation of various medical record software, hardware, word-processing, and database software programs.
