
Patient Support Specialist

Job Overview
As a Social Worker, you will play a vital role in supporting patients with physical conditions through assessment, treatment planning, and therapeutic intervention.
Key Responsibilities:
Evaluate patients' physical conditions through observation, interviews, and diagnostic tests to identify areas of concern.
Develop personalized treatment plans based on assessment findings and patient goals, incorporating evidence-based practices and therapies.
Administer a range of physical therapy techniques, including exercises, manual therapy, electrotherapy, and mobility training to promote optimal patient outcomes.
Help patients manage acute or chronic pain through non-pharmacological approaches, such as education, relaxation techniques, and lifestyle modifications.
Assist patients in recovering mobility and strength post-surgery, injury, stroke, or neurological conditions by providing tailored rehabilitation support.
Track and document patient progress, making adjustments to treatment plans as needed to ensure effective care.
Teach patients and caregivers about home exercises, posture correction, ergonomic practices, and injury prevention strategies to promote independence and self-care.
Collaborate closely with healthcare professionals, including doctors, nurses, occupational therapists, and caregivers, to provide holistic care and support.
Operate and guide patients in using physiotherapy tools and assistive devices, such as walkers, braces, or balance boards, to enhance mobility and function.
Maintain accurate records, prepare reports, and contribute to case discussions or documentation as required to ensure seamless communication and quality care.
Requirements:
Master's degree in Social Work (MSW) or related field.
Current licensure or certification in Social Work (LCSW or LSW).
Excellent communication and interpersonal skills.
Ability to work effectively in a team environment.
Strong analytical and problem-solving skills.
Proficiency in electronic medical records and other relevant software applications.
