Role Summary

We are seeking a Medical Coder to join our team. As a Medical Coder, you will play a key role in ensuring the accuracy and quality of coded data.

Description:

  • Review clinical documentation to assign diagnosis and procedure codes using ICD-10-CM, CPT guidelines.
  • Conduct risk adjustment/HCC coding to identify chronic conditions accurately.
  • Perform chart audits to evaluate documentation insufficiencies.
  • Ensure coded data adhere to payer/regulatory guidelines and internal standards.
  • Resolve queries with providers when medical records are unclear.

Key Responsibilities:

  • Coding Review: Review clinical documentation to assign appropriate diagnosis and procedure codes using ICD-10-CM, CPT guidelines.
  • Risk Adjustment Coding: Conduct risk adjustment/HCC coding to identify and code chronic conditions accurately.
  • Chart Audits: Perform chart audits to evaluate documentation insufficiencies and ensure they support code assignments.
  • Data Integrity: Ensure that coded data adhere to payer/regulatory guidelines and internal compliance standards.
  • Query Resolution: Resolve queries with providers or documentation authors when medical records are unclear or missing needed details.

Requirements:

  • Certification: CPC and/or CRC from recognized bodies.
  • Education: Graduate preferably in Life Sciences.
  • Experience: Typically 1-3 years coding experience in U.S. healthcare.
  • Skills: Strong knowledge of medical terminology, anatomy & physiology.
  • Coding Systems: Proficiency with ICD-10-CM, CPT, and other coding systems.
