We are looking to recruit several enthusiastic, motivated Advanced Nurse Practitioner (ANP) with excellent communication and clinical practice skills to work within the community setting. The Hospital at Home (H@H) service provides secondary level care within the patient's own home or care setting. The ANP within the H@H team plays a key role in the management of acute and complex patients, providing clinical care to patients who would otherwise be in a hospital setting. This post will be part of the nursing team expecting the ANP to undertake staff development and supervision responsibilities. These responsibilities have been reflected in the key results and will apply as required. The post holder will have the support of a senior medical clinician to enhance the ongoing development of the team and staff members. Currently the service is 7 days, 0800-1800, the post holder would work Monday-Friday with weekend working.
