Job Description

Apollo Tele Health Services is seeking a dedicated Community Health Worker to support public health initiatives by engaging with local communities, promoting wellness, and improving access to healthcare services.

Key Responsibilities:

- Conduct outreach activities within communities to raise awareness of health programs and services.
- Educate individuals and families on health topics, disease prevention, and healthy lifestyle choices.
- Assist patients in navigating healthcare systems, including scheduling appointments and accessing social services.
- Act as a liaison between healthcare providers and the community to ensure effective communication and care coordination.
- Identify community needs through field visits, interviews, and assessments.
- Provide informal counseling and support to individuals and families in need.
- Collect, maintain, and report data on community health trends and program impact.
- Support immunization drives, health camps, and public health screenings.
- Advocate for underserved populations and connect them with appropriate resources.
