**General Purpose of Job**:
Provides support for the case management area in the Patient Support Services Department. Facilitates and coordinates referrals for post-acute services, facilities, transfers and/or transportation to alternate care settings. Obtains authorization and works with the health care payers for post-acute services while under the direction of the Medical Social Worker / RN Case Manager. Ensures timely distribution of the regulatory mandated discharge notices to patients and/or patient representatives. Interacts with patients, families, and the health care team to ensure optimal discharge and customer service experiences.

**Essential Duties**:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Collaborates with the Medical Social Worker / RN Case Managers and the interdisciplinary healthcare team in the early identification of patients with discharge planning needs.
- Communicates effectively with post-acute facilities to facilitate patient placement while maintaining a strong working relationship with the various facilities.
- Works collaboratively with the Medical Social Worker / RN Case Manager to obtain authorization for post-acute services.
- Determines the appropriate post-acute care facility for patients who are discharging, and submits the referral based on identified discharge needs.
- Tracks each submitted referral to the appropriate completion.
- Independently organizes and sets priorities based on ever changing workload, conditions, and in a fast-paced environment.
- Initiates, delivers, and ensures regulatory required Notices are distributed to patients, and memorialized in the record.
- Other duties as assigned under the guidance of the Medical Social Worker / RN Case Manager.

**Work Experience**
- Minimum of one year of direct patient care experience.

**Education**
- Associate degree with completion of an accredited social worker program. **Bachelors degree in Social Work. Preferred**

Specialized Knowledge and Skills
- Successful completion of a medical terminology test 80% or higher.
- Ability to work independently and practice excellent organization and priority setting skills to assess urgency and complexity of referrals.
- Excellent interpersonal skills necessary to effectively interact with physicians, social workers, RN case managers and community-based agencies.
- Basic understanding of discharge planning for hospitalized patients and long-term care.
- Knowledgeable about the care of neonates, pediatric, adolescent, adult, and geriatric patients.
- Computer skills including electronic health records and Microsoft office (Word, Excel, PPT), and basic office equipment.
- Demonstrates the ability to work collaboratively across the health care continuum with a variety of people.
