We are looking for a Nursing Coordinator to join our team to asses our customer clinically and build individualized care plans for our customers.

Care Manager responsibilities include collaborating with the family members, monitoring patient recovery, and helping patients by providing appropriate care giver to fullfill their care needs in the care plan.

Ultimately, you will be a bridge between the customer, organisation& caretaker & you will help the customer to improve their quality of life.

**Responsibilities**
- Interact with healthcare workers to see the how care is delivered according to the care plan.
- Ensure patients’ care requirements are met
- Instruct and educate patients on procedures, healthcare provider instructions, and referrals
- Conduct regular follow-ups with patients to evaluate progress, promote continuity of quality care, and ensure improved health outcomes
- Maintain records of care management activities

**Requirements and skills**
- Proven work experience as a Registered Nursing Staff in Hospital.
- Good communication skills,
- Organizing skills
- Willing to travel, two wheeler is must

**Salary**: ₹18,000.00 - ₹30,000.00 per month
