**Care Manager’s Role. ( TRAINING WILL BE PROVIDED)**

As a Remote Care Manager, you will work with your Care Coordinator to ensure that your assigned patients will have appropriate care Plan.

You will review Patient electronic Medical Record (chart) online.

You will review Patient’s progress Notes and create a Nursing Note based on the Physician Notes and findings.

Provides clinical expertise and oversight for chronic care management of patients. Acts as a resource to others.

Is an expert in the use of the electronic documentation and provides consultative support and training to staff in required elements, documentation and coordination.

Ensures that the electronic comprehensive care plan is created and maintained accurately per Medicare

Establishes and maintains positive relationships with all internal and external customers. Acts as a liaison for assigned patients with client (physician office, facility, etc.).

you will be responsible for:

- Documenting care provided, assessments reviewed, goals set, and patient progress in the Portal
- Documenting time spent with on clinical review of patient chart
- Communicate effectively with patient, your Care Coordinator, medical/dental providers, and specialists to collect and verify all required member specific data
- Escalating patient issues as needed in accordance with the - escalation practice
- Ensure continuity of care with a designated practitioner or member of the care team with whom the patient is able to get successive routine appointments.
- Receiving, retrieving / collecting patient data from - CCM Portal / patient EMR system and from various doctors’ offices via telephonic / Fax communication
- Processes and enters all information and relevant data accurately and promptly into electronic medical record.
- comprehensive Patient care plan according to - processes and procedures along with time-stamping.
- Creates patient specific wellness plan.
- Care Managers are experienced in complex care coordination assessments
- Care Manager monitors beneficiary’s condition and updates Care Management activities in portal
- Performs ongoing medication adherence and reconciliation
- Ensures beneficiaries preventative services

TRAINING WILL BE PROVIDED

**Job Types**: Full-time, Part-time
Part-time hours: 40 per week

Pay: From ₹25,000.00 per month
