Roles and responsibilities includes
- **Record Management**:
Create and maintain electronic or paper-based health records for patients.

Ensure the accuracy, completeness, and confidentiality of patient information.

Organize and file medical records according to established procedures.
- **Coding and Indexing**:
Assign appropriate codes to diagnoses and procedures using standard coding systems (such as ICD-10 and CPT).

Index and classify medical information for easy retrieval and analysis.
- **Data Entry**:
Input patient information into computer systems, updating records with new information.

Verify the accuracy of entered data to maintain the integrity of health records.
- **Release of Information**:
Process requests for patient information from healthcare providers, insurance companies, and other authorized entities.

Ensure compliance with privacy laws and regulations when releasing patient records.
- **Quality Assurance**:
Regularly audit and review health records to ensure they meet quality and regulatory standards.

Identify and correct any discrepancies or errors in patient records.
- **Collaboration**:
Work closely with healthcare professionals, including physicians, nurses, and administrative staff, to obtain and clarify information for accurate record-keeping.
- **Technology Utilization**:
Use electronic health record (EHR) systems and other health information management software to manage and retrieve patient data.

Stay updated on technological advancements in health information management.
- **Compliance**:
Adhere to privacy and security regulations, such as the Health Insurance Portability and Accountability Act (HIPAA), to protect patient confidentiality.

Stay informed about changes in healthcare laws and regulations related to medical records.
- **Patient Education**:
Educate patients about their rights regarding access to their medical records and the release of information policies.
- **Reporting**:
Generate reports and statistical information from health records for administrative and research purposes.
- **Training and Supervision**:
Train and supervise staff involved in medical records functions, ensuring they follow established protocols.

**Salary**: ₹15,000.00 - ₹20,000.00 per month
