Social workers provide assessment and therapeutic intervention for patients throughout their Palliative Care journey. The patients we see present with a range of psychosocial issues that impact their health during their end-of-life care. The Social Worker will understand issues related to health and wellbeing and the impact on the person and their family/carer, including adjustment to change in their health.  The Social Worker will promote improved client outcomes through working in collaboration with the multidisciplinary team to provide high quality Specialist Palliative Care services.
