Diabetes Nurse Coordinator/Educator

Job Details
  • Shift: 40 hours per week, Monday- Friday
  • Work Location: Onsite, Palo Alto, CA
Duties and Responsibilities
  • Collaborates with nursing, interdisciplinary teams and community resources to define problems, develop
  • Plans and provide appropriate referrals.
  • Develops, implements and evaluates cost-effective educational programs for diabetic patients, their families and communities.
  • Identifies specific needs for, and means to, enhance professional development and maintain clinical competency.
  • Incorporates clinical research in clinical, educational and consultative roles to provide a scientific basis for  professional nursing practice.
  • Knowledge of glucose monitoring systems, insulin administration technology, pharmacologic and nonpharmacologic treatment of diabetes and complications.
  • Promotes quality patient care by serving as a resource to care providers of patients with diabetes
  • Provides and role models indirect care of diabetic patients.
  • Teaches patients, families and staff in formal and informal settings about the care needs of diabetic patients using appropriate methodology and strategies

Requirements Education Qualifications
  • Bachelor’s Degree or higher in Nursing from an accredited academic institution. (preferred)

Licenses and Certifications
  • Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure or
  • CDCES - Certified Diabetes Care and Education Specialist required
Experience
  • Two (2) years of progressively responsible and directly related work experience

Requirements
