Northern Health are seeking applications from motivated and suitably experienced Allied Health Assistant Grade 3  to join the Transition Care (TCP)/GEM at Resi team. Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north. Services are provided through our six main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital.  In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg. About the Role: The Allied Health Assistant works closely with the multidisciplinary team to provide a flexible and responsive service that works towards meeting the older persons identified needs and goals. This role is responsible for: Contributing towards an interdisciplinary approach to optimising the well-being and function of each individual. Coordinate activities, such as exercise groups Attend visits to the older persons home or bed based care setting. Implement individual and group participation in the Transition Care Program in conjunction with multiple disciplines including dietetic, occupational therapy, physiotherapy, and speech pathology. Management of equipment – including coordinating rental equipment. To be successful in this role you will have: Experience and good working knowledge of the role of the Allied Health Assistant – with a multi-disciplinary skillset (physiotherapy, occupational therapy) Proven ability to ability to work within a team environment Demonstrated ability to follow instructions Well-developed understanding of the roles and contributions of different disciplines to client care and discharge planning.
