Requirements

Job Summary: The Wellness Nurse will play a pivotal role in our wellness program, utilizing their critical care expertise to assess, plan, and implement wellness strategies that cater to the diverse needs of our community. This role involves direct patient interaction, health education, and collaboration with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:

  • Patient Assessment and Care:
    • Conduct comprehensive health assessments and screenings.
    • Develop individualized wellness plans based on patient needs and goals.
    • Provide direct nursing care and health counseling to clients.
  • Health Education and Promotion:
    • Educate clients on health maintenance, disease prevention, and healthy lifestyle choices.
    • Lead wellness workshops, seminars, and support groups.
    • Create educational materials and resources on various health topics.
  • Collaboration and Coordination:
    • Work closely with multidisciplinary teams to coordinate care.
    • Refer clients to appropriate healthcare providers and resources as needed.
    • Act as a liaison between clients and healthcare services.
  • Program Development and Implementation:
    • Assist in the development and evaluation of wellness programs and initiatives.
    • Monitor and track the progress of wellness programs and make adjustments as necessary.
    • Stay updated with the latest health trends, research, and best practices.
  • Documentation and Compliance:
    • Maintain accurate and confidential health records.
    • Ensure compliance with all healthcare regulations and standards.
    • Participate in quality improvement activities and initiatives.

Qualifications:

  • Education and Experience:
    • Bachelor’s degree in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
    • Current RN license in good standing.
    • Minimum of 2 years of ICU experience required.
    • Previous experience in wellness or community health nursing is preferred.
  • Skills and Abilities:
    • Strong clinical skills with the ability to handle high-stress situations calmly and effectively.
    • Excellent communication and interpersonal skills.
