Job Title: Service/CareCoordinator

Location: Remotein TX

Duration: 6 Months(Possible Contract to hire)

Shift Hours: Monday Friday 8:00 AM 6:30 PM(CST)

Summary:

  • Provide support to medical behavioral and social supportstaff to determine needs eligibility and services formembers.
  • Conduct screening and risk assessmentinterventions per program guidelines.
  • Candidate must be in TX/ Preferred El Paso TX

  • These positions work in a Call Center setting where thecontractors are on a phone queue.
  • Day to Dayresponsibilities Inbound call center assist and educate Medicaidmembers on benefits and Services such as PCP Changes ID Cards andCoordinate transportation and other services.

Describe the performanceexpectations/metrics for this individual and theirteam:

  • Adherence 90% orabove
  • Quality 95% or above

  • Day 1/week 1:Issued password badging laptop equipment ensure systems access andwill start University training throughout the firstweek.
  • Training: After week 1 will be set upwith trainer for daytoday visibility into the role.

JobResponsibilities:

  • Identifyspecial needs members through the completion of health screens andother resources.
  • Work with communityoutreach/member advocates to coordinate member care.
  • Educate providers and community resources on programcomponents and available support services.
  • Educate members with special needs to foster compliancewith program and positively impact outcomes.
  • Conduct site visits as appropriate for programs andprovide support to other special programs.
  • Assist with modification and management of care plansunder the direction of a Registered Nurse in conjunction withmember medical consenter or caregiver Department of Family andProtective Services (DFPS) caseworker and family if applicable aswell as the managing physician.

Qualifications:

  • High School Diploma or GED.
  • MedicalLong Term Services and Supports Background Workingwith the Aging Blind and Disabled Population.
  • Microsoft 365 applications including Outlook Excel Wordetc. Avaya soft phone knowledge.
  • MCO MedicaidSTARPLUS insurance knowledge.
  • Call centerexperience is a must; workers are expected to be on the phone forthe shift and will need to follow their schedule in a timelymanner.

PreferredQualification:

  • Backgroundin Managed care.
  • Preferred BA in Social Workor Healthcare Mgmt. LVN CNA CMA RT Pharmacy Tech.
