Position title:Care Navigator JobLocation: BangaloreWorking Hours: 9hrs a day 6 days a week (Hybrid working hours mandatory to work fewdays in a month from the office) Positionsummary: The Care Navigator plays a crucialrole in providing support to cancer patients and their caregiversthroughout their cancer journey. Serving as the main point ofcontact for all patients enrolled on the platform they need tobuild strong relationships with both patients and caregivers toensure patients remain actively engaged on the app for cancercare The Care Navigator needs to be committedto breaking down barriers to care. these navigators identifycritical resources for patients helping them navigate throughvarious services and offerings. The core responsibility of the CareNavigator is to offer guidance emotional support and carecoordination for all the patients. This entails collaboratingclosely with healthcare teams connecting patients with necessaryresources and ensuring a seamless personalized experiencethroughout their treatment and recovery. This role plays anintegral part in enhancing the overall wellbeing of patients andtheir journey towards positive health outcomes Qualifications: Bachelors degree in nursing social work psychologyhealthcare administration or a related field (Masters degreepreferred) Preferred: Formal education as acancer care navigator Oncology nursing background Experience: Minimum of 2 years of experience working in healthcare inoncology or patient navigation nursing roles home care caregiverroles Strong understanding of cancertreatments medical terminologies and the emotional challenges facedby cancer patients Skills: Excellent interpersonal andcommunication skills to effectively interact with patientscaregivers and healthcare professionalsEmpathy compassion and the ability to provide emotionalsupport to patients in distress Strongorganizational skills to manage multiple patients needs andcoordinate care effectively Problemsolvingabilities to address logistical challenges and patientconcerns Strong proficiency in English andHindi writing and speaking skills

Responsibilities:

  • PatientOnboarding and Engagement:
    • Guide newlyenrolled patients and caregivers through the program setting uptheir profiles explaining the available features and how tonavigate the app effectively
    • Foster awelcoming and reassuring experience during onboarding emphasizingthe benefits of using the app for their cancer journey
  • Patient Monitoring and Adherence:
    • Provide anticipatory guidanceto patients in coping with outcomes oftreatment
    • Maintain regularcommunication with patients and caregivers checking in toensure their continued engagement with the app
    • Address any concerns or barriers to app usage providingsupport and guidance to overcomechallenges
  • Acting as a Bridge between Oncologists and Patient:
    • Facilitate effective communication betweenoncologists and patients conveying treatment updates answeringqueries and addressing concerns
    • Ensureseamless information flow between oncologists and patients forinformed decisionmaking
  • Patient and Caregiver Education:
    • Provide and reinforce education to patients families andcaregivers about side effect management posttreatment care andsurvivorship
    • Collaborate with oncologists inrecommending appropriate educational content from the appsresources ensuring patients receive accurate and relevantinformation aligned with their diagnosis treatment and emotionalneeds
  • EmotionalReadiness and Support:
    • Provide support byactively listening to patients and caregivers feelings fears andanxieties related to their cancer journey
    • Offer coping strategies positive reinforcement andreferrals to physio and psycho health professionals whenneeded
  • Access toCancer Products and Services:
    • Guide patientsin exploring and buying cancerrelated products such as wigsprosthesis and other supportive services like nutrition counsellingemotional counselling and physiotherapy etc. available through theapp
  • Communication andReporting:
    • Maintain accurate records ofpatient interactions and interventions ensuring properdocumentation and reporting
    • Regularlycommunicate patient feedback to the concerned team
