Key Responsibilities:-

• Review and assess Individualized Care Plans (ICPs) for assigned patients
• Conduct outbound follow-ups and handle inbound patient queries with empathy and clarity
• Coordinate with internal teams for seamless care management
• Maintain detailed and accurate documentation of all patient interactions

What We're Looking For:-

• Active NCLEX Certification is a must
• 0 to 1 year of experience (Freshers welcome)
• Excellent spoken and written English communication
• Willingness to work night shifts (US Time Zone)
• Based in or around Mumbai, Navi Mumbai, or Thane
• Basic computer skills and comfort with healthcare documentation
