Job Title: Clinical Pharmacy Counsellor Department:

Clinical Services / Patient Care Location: Freelance Counselling (Flexible hours - as per counsellor's availability)

Reporting To: Medical Director / Clinical Coordinator

The company is a patient-centric renal care initiative committed to empowering individuals living with kidney disease through awareness, education, and personalized clinical support. Their multidisciplinary approach integrates nephrology expertise, technology, and compassionate counselling to help patients "balance the flow" - optimizing kidney health and quality of life.

Position Overview The Clinical Pharmacy Counsellor plays a vital role in supporting patients with Chronic Kidney Disease (CKD), dialysis, or transplant care. The counsellor ensures safe and effective medication use, enhances adherence, educates patients and caregivers, and collaborates closely with the nephrology team to optimize clinical outcomes.

Key Responsibilities

  1. Patient Counselling and Education- Conduct structured medication counselling sessions for CKD, dialysis, and transplant patients.- Educate patients on medication adherence, symptom recognition, and preventive care.- Provide nutrient-drug awareness (e.g., phosphate binders, potassium restriction).- Maintain an empathetic, motivating communication style that builds trust.

  2. Clinical Coordination- Collaborate with nephrologists, nurses, and dietitians to ensure integrated patient management.- Review medication charts for drug appropriateness and dose modification in renal impairment.- Identify and report adverse drug reactions (ADRs) and participate in pharmacovigilance activities.- Support implementation of CKD risk calculators and monitoring dashboards for patient follow-up.

  3. Documentation and Reporting- Maintain detailed patient counselling records and follow-up reports.- Track adherence scores, medication refill patterns, and clinical progress.- Support the preparation of clinical outcome summaries and quality audits.

  4. Awareness and Education Programs- Conduct patient awareness sessions on kidney health, dialysis safety, and transplant medication adherence.- Participate in community outreach programs under the "Know Kidney" initiative.- Contribute to educational materials and counselling modules.

Qualifications- Pharm.D / M.Pharm (Clinical Pharmacy) / B.Pharm with PG Diploma in Clinical Pharmacy- 1-3 years of experience in clinical or hospital pharmacy (preferably nephrology, dialysis, or transplant care).- Strong knowledge of renal pharmacotherapy, drug monitoring, and clinical documentation.- Excellent communication, empathy, and patient engagement skills.
