Role & responsibilities

  • Conduct nutritional assessments of patients based on medical history, clinical data, anthropometrics, and dietary intake.
  • Develop, implement, and monitor individualized nutrition care plans.
  • Collaborate with physicians, nurses, and other healthcare providers to coordinate patient care.
  • Provide nutrition education and counseling to patients and families on medical nutrition therapy for various conditions (e.g., diabetes, renal disease, gastrointestinal disorders, etc.).
  • Document all patient interactions and care plans in accordance with facility and regulatory standards.
  • Participate in multidisciplinary rounds and case conferences.
  • Monitor patients' progress and modify care plans as needed.
