Job Summary

USA Healthcare is hiring a Night Shift Pharmacist for our Chennai operations center to support real-time medication reconciliation for U.S. patients. You'll handle live phone calls with patients—no preparation time—while rapidly navigating U.S.-based EMR systems. This role aligns with U.S. overnight hours (7:00 PM – 3:30 AM IST) and requires lightning-fast clinical accuracy, fluency in American English, and comfort with high-pressure, zero-prep med rec.

Role Overview

You will be the first and only clinical touchpoint for US patients discharged from ERs, hospitals, or SNFs. Your sole KPI: complete a 100% accurate medication reconciliation in under 5 minutes — live on the phone, with zero chart preparation.

This is not a dispensing or counseling role. This is high-velocity clinical triage:

• Patient calls in You pick up EMR opens simultaneously You build, verify, and finalize the med list while the patient is still speaking.
• No hand-offs. No delays. No "I'll call you back."

Core Responsibilities

1. Zero-Prep Live Med Rec (75% of shift)

• Answer inbound calls within 3 rings (AHT less than 4:45).

• Simultaneously:

• Open patient chart in US EMR (Epic/Cerner/Meditech – dual 27" monitors).

• Greet patient in neutral American English.

• Begin verbal med rec while typing.

• Ask 5 high-yield questions (proprietary script):

1. "What meds are you holding in your hand right now?"

2. "Any pills from the hospital bottle?"

3. "OTC, supplements, or PRNs?"

4. "Allergies or reactions?"

5. "Who filled your last Rx?"

• Cross-verify in less than 60 seconds with:

• Discharge summary (PDF in EMR)

• Surescripts Rx history

• State PDMP (if flagged)

• Flag & resolve red-alert discrepancies:

• Duplications (e.g., two opioids)

• Omitted home meds (e.g., apixaban)

• Dose/form errors (e.g., 5 mg vs 50 mg)

• Finalize & lock med list e-sign route to US MD for co-signature.

2. EMR Mastery (15% of shift)

• Navigate 3 EMRs simultaneously (training provided):

• Eclinical Work (80% volume) – Hyperspace + SmartTools

• Cerner – PowerChart

• Meditech – 6.1x

• Use keyboard shortcuts only (mouse = disqualification).

• Document in structured fields:

• Med name | Strength | Dose | Route | Freq | Last dose | Source (Pt/DC/Rx)

3. Escalation & Handoff (8% of shift)

• Immediate escalation (within 15 sec) for:

• Controlled substance mismatch

• Life-threatening omission (e.g., insulin, AC)

• Patient confusion/delirium

• Complete SBAR handoff note in less than 90 sec.

4. Quality & Compliance (2% of shift)

• 100% of calls dual-recorded (QA + training).
• Daily audit: 1 random call reviewed live by US Lead.
• Maintain less than 0.5% error rate (critical discrepancy = termination).

Required Qualifications

Education & Licensing

• B.Pharm / M.Pharm (PCI-approved college)
• Active Tamil Nadu Pharmacy Council registration (renewed)
• US Telepharmacy Credentialing (we sponsor within 30 days)

Clinical Experience

• 2+ years in hospital inpatient / discharge pharmacy
• Proven ability to reconcile 15+ meds in less than 5 min (live demo required)
• Experience with US drug nomenclature (e.g., "Lisinopril" not "Lisinoprilum")

Technical Skills

• Typing speed: 70 WPM (with 98% accuracy)
• EMR proficiency: Able to complete mock med rec in Epic sandbox in 4:30
• Dual-monitor workflow: Muscle memory for Alt+Tab, Ctrl+F, etc.

Language & Communication

• Native-level American English (neutral accent – no MTI)
• Able to mirror US patient tone: calm, empathetic, authoritative
• Script deviation allowed only for clinical urgency

Availability

• Fixed night shift (7:00 PM – 3:30 AM IST) – no swaps
• Willing to work US Thanksgiving, Christmas, July 4
