Job Summary
The Home Care Nurse provides skilled nursing care to clients in their homes. This includes clinical assessments, medication management, wound care, monitoring chronic conditions, and supporting families with education and guidance. The nurse ensures safe, high-quality care while promoting dignity, comfort, and independence.
Key Responsibilities
Clinical Assessment & Care
- Conduct initial and ongoing nursing assessments at the client's home.
- Monitor and record vital signs, symptoms, and health changes.
- Administer medications, injections, and IV therapies (if applicable).
- Provide wound care, catheter care, stoma care, and other nursing procedures.
- Manage chronic diseases such as diabetes, hypertension, stroke, COPD, etc.
- Handle emergencies and perform first-response interventions.
Care Planning & Coordination
- Develop and update individualized home-care plans.
- Coordinate with physicians, therapists, families, and the internal care team.
- Guide caregivers and family members on proper care techniques.
- Ensure smooth communication and handovers between shifts or visits.
Patient & Family Support
- Educate clients and family members on health conditions, medications, equipment use, and home safety.
- Provide emotional support and promote independence as much as possible.
- Respect client privacy, dignity, and cultural preferences.
Monitoring & Documentation
- Maintain accurate and timely nursing records, progress notes, and care updates.
- Document medication administration and procedures performed during visits.
- Report changes in condition or safety concerns to the supervisor immediately.
Safety & Compliance
- Follow infection-control and home-care safety protocols.
- Assess home conditions and recommend improvements for safety (e.g., fall prevention).
- Ensure adherence to company policies and healthcare regulations.
Qualifications
- Registered Nurse (RN) license (valid and current).
- Nursing Diploma or Bachelor's Degree.
- Experience in home care, elder care, community nursing, or hospital settings preferred.
- Knowledge of home-care equipment (suction machine, oxygen, nebulizer, glucometer, BP monitor, etc.).
- Strong communication, critical thinking, and independent decision-making skills.
- Empathy, patience, and ability to work with minimal supervision.

- Ability to train caregivers and families on basic care.
Working Conditions
