Role & responsibilities

Preferred

  1. Medication Management

  2. Ensure accurate dispensing of medications, especially for intravenous and chemotherapy preparations.

  3. Monitor drug therapy regimens for interactions, contraindications, and appropriateness.

  4. Maintain proper storage conditions and inventory control of all medications.

  5. Compliance & Safety

  6. Ensure adherence to hospital policies, procedures, and legal regulations.

  7. Conduct regular audits and inspections of drug administration records and medication areas.

  8. Promote safe drug disposal practices and pharmacovigilance.

  9. Coordination with Healthcare Teams

  10. Collaborate with doctors, nurses, and technicians to ensure proper medication administration.

  11. Provide drug-related information, including side effects, dosage, and interactions.

  12. Assist in selecting appropriate drug regimens and scheduling administration times.

  13. Patient Care & Education

  14. Take patient medication histories and counsel them on drug usage and side effects.

  15. Monitor patient responses to medications and adjust therapy as needed.

  16. Participate in discharge planning by reviewing medication charts and summaries.

  17. Supervision & Training

  18. Oversee pharmacy technicians and interns.

  19. Conduct in-service training for pharmacy staff and nursing personnel.

  20. Ensure proper compounding techniques and documentation practices.

  21. Record Keeping & Documentation

  22. Maintain accurate records of drug supply, billing, and investigational drugs.

  23. Ensure prescriptions are correctly entered into the unit dose system.

  24. Periodically review and update patient medication charts.

  25. Emergency Response

  26. Participate in cardiopulmonary and other medical emergencies by providing rapid pharmaceutical support.

  27. Ensure availability of emergency medications and supplies.
