*Position:*
An exciting opportunity has arisen to recruit two Primary Care Health Care Support Workers (HCSW) within our Community Treatment Care (CTAC) service based in General Practices within South Health and Social Care Partnership working as part of an integrated nursing team model and wider General Practice multidisciplinary team
*What You Will Do:*
You will be responsible for supporting the delivery of quality person centred nursing care and defined nursing interventions, primarily in a Community Treatment and Care Room Setting, to a practice population and the wider locality.

*Knowledge, Training And/or Experience Required To Do The Job:*
Community/General Practice experience is desirable but not essential as learning required to fulfil the responsibilities of the post will be supported in practice. We welcome applicants from HCSW interested in a career in Primary/Community Care.

*Hours:*
37 hours per week working Monday to Friday
14.8 hours per week working Monday to Friday
