Primary Job Title:
Home Health Nurse (Home Care Staff Nurse)

Role & Responsibilities

  • Provide direct nursing care in patients' homes: perform clinical assessments, administer medications, IV therapy, wound care, catheter and stoma care as prescribed.
  • Develop and implement individualized care plans after conducting comprehensive home health assessments; update plans based on progress and clinician directives.
  • Monitor vital signs and clinical parameters, document findings accurately in electronic medical records, and escalate clinical concerns to supervising physicians or care coordinators.
  • Deliver patient and family education on medication management, wound care, mobility, infection prevention, and discharge instructions to support self-care and adherence.
  • Coordinate multidisciplinary care: liaise with physicians, physiotherapists, pharmacists, and social workers to ensure continuity and quality of care.
  • Adhere to infection control protocols, maintain medical equipment and supplies, and contribute to quality improvement and safety reporting.

Skills & Qualifications
Must-Have

  • Wound care and dressing management
  • Intravenous (IV) therapy and cannulation
  • Medication administration and dosage calculation
  • Home health assessment and individualized care planning
  • Infection control and aseptic technique
  • Basic Life Support (BLS) certification

Preferred

  • Palliative and end-of-life care
  • Chronic disease management (diabetes, COPD, heart failure)
  • Experience with electronic medical records and remote monitoring tools

Qualifications

  • Registered Nurse qualification: BSc Nursing, GNM, or ANM (as applicable) with valid state nursing registration.
  • Proven experience in home healthcare, community nursing, or hospital bedside care preferred.
  • Current BLS certification; additional certifications (Advanced Cardiac Life Support, wound-care courses) advantageous.
  • Willingness to travel to patient homes across assigned regions and work a hybrid schedule (field visits + telehealth follow-ups).
