  • Conduct psychosocial assessments of patients and their families.
  • Develop and implement individualized care plans in collaboration with psychiatrists, psychologists, and other healthcare professionals.
  • Provide counseling, psychoeducation, and support to patients and caregivers.
  • Facilitate access to community resources, social services, and rehabilitation programs.
  • Conduct home visits and community outreach for follow-up and support.
  • Advocate for patients' rights and mental health awareness in the community.
  • Maintain accurate records of patient interactions and progress.
  • Organize and participate in mental health awareness campaigns, support groups, and workshops.
  • Collaborate with governmental and non-governmental organizations for patient support and rehabilitation.
Requirements
  • Strong counseling and communication skills.
