**Job Summary**:
The Home Care Nurse provides comprehensive nursing care to cancer patients in their homes. The nurse works closely with patients, families, and healthcare teams to deliver personalized care, manage symptoms, and improve the quality of life for those undergoing cancer treatment or palliative care.
**Key Responsibilities**:
**Patient Assessment**:
- Conduct thorough assessments of patients' physical, emotional, social, and environmental needs.
- Monitor patients' symptoms and responses to treatments.
- Evaluate the effectiveness of pain management and adjust care plans as necessary.
**Care Planning and Implementation**:
- Develop and implement individualized care plans based on patient assessments.
- Administer medications, including chemotherapy, and monitor for adverse reactions.
- Provide wound care, nutrition support, and other specialized treatments as needed.
**Education and Support**:
- Educate patients and families about cancer, treatment options, and side effects management.
- Offer emotional support and counseling to patients and their families.
- Coordinate with community resources to provide additional support and services.
**Communication and Coordination**:
- Collaborate with oncologists, primary care physicians, and other healthcare professionals to ensure coordinated and comprehensive care.
- Maintain detailed patient records and communicate updates to the healthcare team.
- Facilitate transitions between healthcare settings if hospitalization is required.
**Symptom Management**:
- Monitor and manage symptoms and side effects of cancer and its treatment.
- Provide palliative care to improve the quality of life for patients with advanced cancer.
**Qualifications**:
- Current RN or LPN license.
- Bachelor's or associate's degree in nursing.
- Experience in oncology nursing or home health care is desirable.
- Strong communication and interpersonal skills.
- Ability to work independently in a home setting.
- Compassion and sensitivity to the needs of cancer patients and their families.
Pay: ₹10,000.00 - ₹13,000.00 per month
