Knowledge: Current practices and guidelines in General Medicine

Management of common clinical problems in chronic kidney disease

Management of medical emergencies including use of drugs in patients with medical

emergencies and

chronic kidney disease and their doses

Current practices and guidelines in Nephrology during the course

Skills: Procedures related to General Medicine (including management of unresponsive

patients with

basic life support and advanced cardiac life support)

Training and gaining experience in procedures related to Nephrology

Communication with patients and Hospital staff at other levels

Reporting to: Head, Department of Nephrology; Medical Director; Unit Head; Nephrology

consultants; Senior registrar

Reported by: Duty doctors, dialysis medical officer, physician assistants, Dialysis staff,

Nephrology

nurse; indirect reporting by Nursing Staff, public relationship manager, Housekeeping

Staff

Key result areas:
Coordination with the consultant(s) of department of Nephrology and other departments

for delivery

of safe effective compassionate and quality care to the patients

Participation in discussion and management of patients on a day to day basis

Implementation of hospital protocols and policies in general and with respect to

Nephrology practice

in particular

Compliance with patient care related safety policies, patient rights related policies and

NABH

guidelines

Adherence to code of medical ethics

General responsibilities:
History taking and physical examination of newly registered patients in outpatient

department with

appropriate documentation before Nephrology consult

History taking and physical examination of previously registered patients in outpatient

department

with appropriate documentation and feedback to the consultant or registrar

Clinical assessment of all patients presenting to emergency ward, hemodialysis unit and

patients

getting admitted to wards with appropriate documentation

Regular assessment of all inpatients admitted under Nephrology during ward rounds and

during duty

hours with appropriate documentation

History taking, physical examination and regular assessment of all in patients admitted in

Intensive

Care Unit with appropriate documentation

History taking, physical examination and assessment of inpatients admitted under Urology

referred

for management of general medical and nephrology problems with appropriate

documentation

Immediate and appropriate management of emergencies in inpatients, outpatients and

patients in

hemodialysis unit if detected during clinical assessment with urgent feedback to the

concerned

consultant and appropriate documentation

Management of unresponsive patients with basic life support and advanced cardiac life

support

Appropriate follow up of investigation reports and presentation to the concerned

consultant

Feedback to patients and attendants on reports, action taken if any and plans of action

following

discussion with the concerned consultant

Documentation of informed consent

Patient procedures (intra venous cannulation, arterial puncture for collection of blood

sample for gas

analysis, nasogastric tube insertion, pleural effusion and peritoneal fluid aspiration for

diagnostic and

therapeutic purposes, non tunneled and tunneled central vein catheter insertion, renal

biopsy,

percutaneous insertion of peritoneal dialysis catheter)

Proper handover of patient details for continuity in treatment during shift change

Specific responsibilities:
Work ethics - reporting for duty, duty roster, coordination with colleagues

Academic meets (including dialysis and transplant) of the department

Appropriate documentation and maintenance of log

Performance of research activities - dissertation: ideation, collection of materials and

methods,

analysis, documentation and submission; clinical research on a particular theme at least one

a year for

presentation in clinical meets inside and outside Hospital and in regional National and

International

meets

Publication of the above research activities

Performance in assessment activities of the Board

Morbidity and Mortality meets

Clinical audit meets

Cardiopulmonary resuscitation committee meets

Academic forums and meets (including regional, national and international conferences) as

and when

required

Registration with Karnataka Medical Council
