**Roles and Responsibilities**

**DUTIES & RESPONSIBILITIES OF STAFF NURSE**

Staff Nurse is a First Level Professional Nurse who provides direct patient care to one patient or group of patients assigned to her/him during duty Shift and assists in management of wards. She/he is directly responsible to Senior Staff-Nurse of ward in-charge Nurse/NS

Key Responsibilities of Staff Nurse

1) To ensure cleaning & dusting.

2) Medication administration & collection.

3) **Patient Care**: (i) Dress changing. (ii) Oral care. (iii) Back care. (iv) Bowl & Ladder Care.

4) Monitoring patient vitals & urine output or dry the drain output.

5) Feeding & monitoring of feeding (Supplementation and nutritional therapy)

6) Staff Education.

7) Accompanying infection during rounds.

8) Implementing infection control practices.

9) Maintaining Air Mattresses.

10) To maintenance of intake & output chart.

11) Observes change in patients conditions & records.

12) Collection & sending specimen pre & post-operative care assists in administration of transfusion, perineal care, baby care etc.

13) To complete admission and discharge formalities of the patients after permission of the doctor.

14) It is also mandatory to remove by the means of transfer out/discharge/LAMA/death immediately so that false data may not display on dashboard.

**Direct Patient Care**

1. To carry out the procedure of admission and discharge of the patient.

2. To arrange proper bed for patients and helps or guides students or Ward Boy & Ayas to make beds properly.

3. To maintain Hygiene and comforts of the patients.

4. To maintain clean and hygiene environment of the ward.

5. To ensure the fulfillment of the nutritional needs of the patients.

6. To maintain ward policies and routines.

7. To take round with the doctors and write down new instructions given by them and to follow instruction and prescription about patient given by the Doctors.

8. Perform various technical tasks restated to nursing care.

9. Administering of Medicine i.e. Tables, Injections, Infusion and Transfusion as per doctors instruction.

10. Keep recording of vital sign, temp, pulse, resp. & B.P. etc.

11. To prepare patient for operation and to ensure that patient is sent to O.T. on time with all necessary papers and medication.

12. To take care of Ears, Back, Bowl, Bladder perineum and Bracts and paralyzed patient to prevent bed sore.

13. To provide bed side nursing care to all patients.

14. Attend last officers in case of a patient dying during shift and arrange to preserve dead body in mortuary or hand over the body with respect to concerned family members/relatives.

15. To keep updated patients file.

16. Follow aseptic measures to prevent hospital acquired infection.

17. To ensure proper disposal of Bio-medical waste as per B.M.W. management and handling rules

18. To provide Food to patient as per direction given by consultant and dietitian.

**Ward Management**

1. To maintain General cleanliness of the ward and the sanitary Annexure.

2. To supervise the work of wards boys and Ayas and guide them. If they are not working properly inform to N.S.

3. To supervise and guide interns about nursing care and other work carried out in the ward.

4. To maintain duty room, emergency trays, Sterilizer Instruments, and to ensure procedure trays ready condition.

5. Before taking off charge of ward you should check all instruments, emergency tray etc. And do before handover the charge to next staff.

6. To maintain all the records pertaining to ward.

7. To take a special care of Medico-Legal case (MLC) papers and records.

8. To Maintain Death Register and Brought Dead Patient Record Register.

9. Writes Day and Night Doctors orders and maintain ward statistics.

10. To maintain case papers, Investigation Reports.

11. Any other work as assigned by the management.

12. To maintain/Record and Report manually any HIS Portal.

In Absence of Technician Staff Nurse to perform following duties:
1 Inotropic / vasopressor / Norad - (Tapering/Increase)

2 Helping in CVP line.

3 Arterial B.P monitoring.

4 Patient O2 taper/Increase manually and through device provided by Biomedical Engineer.

5 Take critical patients to MRI/CT Scan with all precautions.

6 Intake and output by or device given by Biomedical Department.

7 In Absence of Staff Nurse - RT feed.

8 To perform Technician duty in code blue and RRT.

9 Nutritional Therapy and Supplementation.

Bachelor in Nursing (B.Sc. Nursing) or Diploma in Nursing (GNM)
