**RESPONSIBLITIES OF THE CLINICAL PHARMACIST**

**Qualifiacation: Pharma D.**

**Gender : Female Only**

**Experience: Minimum 1 year**
- Check all discharge summaries and patient files for medication duplication and interactions. Coordinate with the EMRD staff to complete discharge summary on time.
- Audit patient files for any dosage error, dosage adjustment (Pediatric, Renal Problem, Hepatic problem, Obese) Administration error, Dispensing error, Prescription error.
- Clear all queries from the staffs (Nurse, Pharmacists) regarding medications dilutions, storage, duration of administration, route of administration.
- Coordinate with Pharmacy staff and arrange alternative drugs which is not available in pharmacy advised by the consultants.
- Coordinate with Pharmacy staff to get medication as per discharge summary and explain bed side all drugs and their usage and precaution/ side effects.
- Accompany consultants on rounds and make necessary changes in treatment chart as per instructions given.
- Audit treatment chart and nurses record to verify correct dose and timings of medications.
- Go on rounds and see if patients have any queries with regards with their treatment plan.
- Audit ward stock on monthly basis.
- Complete the ongoing hospital formulary work and submit to the consultant.
- Suggest improvement to the new software being developed for medication indenting and documentations.

**Salary**: ₹18,000.00 - ₹35,000.00 per month
